Is there a certain shape to the kama when laid flat? Are the sides tapered inward, outward, or are they just straight? It seems to be bigger around the waist than around the knees.

It is indeed bigger around the waist than at the knees.
Basically, when laid flat, it should be longer at the top and shorter at the bottom, so you're basically looking at a top-heavy trapezoid with a wedge cut out of the bottom center. The look is really specific, so you actually better measure for your own body on this one. I'd give you general measurements, but they're kind of difficult.
Basically, the only measurement I can give you is this: The space between the front top corners should be the same distance from the center of one belt buckle to the center of the other. Use the Sith Snapshot #1 for that measurement reference.
For the bottom length, when you are cutting your material, cut it out as a big RECTANGLE, and then put it on. Mark it where the cloth is at the side of your knee. The bottom corners are beside your knee.
It should be tall enough that the bottom edge is just below the knee armor, or at the top of the shin armor.

Awesome, thanks Cap! Sounds like I'll need to hold off on making the kama till I have knees or shins, or make a really good educated guess.
